Changing dependants
You may add or remove dependants at any time during the year. New spouses and newborn babies added within 30/90 days of the event can join without underwriting (no waiting periods) whereas other new dependants may receive underwriting.
Special dependants e.g., parents may only be added if they are financially dependent on the main member and subject to the medical scheme rules and your employer policy.

Changing your option
Momentum Health allows specified changes, such as changing from ‘State’ chronic to ‘Associated/Any’ chronic within 30 days’ of diagnosis of a new chronic condition (certain options only). Specified downgrades may also be allowed during the year, subject to scheme approval. It is important to note that moving to a lower plan may result in an amount owing to the scheme if you have spent more from your Personal Savings Account than the amount contributed at the time of the downgrade.
All other upgrades and downgrades and scheme changes are only allowed at the end of each year effective 1 January of the following year. See the Year End Option Changes Card
Chronic medicine benefit
If you have been diagnosed with a chronic condition e.g., hypertension, diabetes or high blood pressure, you need to register on the Momentum chronic illness benefit.
To register, please follow the below steps.
- Ask your treating doctor or pharmacist to contact Momentum on 0860 11 78 59.
- The chronic benefit consultant will either approve or decline the benefit telephonically.
- Momentum may need additional information from your provider, such as the ICD-10 code (diagnosis code) and applicable test results, in order to complete the registration process for your condition or medicine.
- Once the chronic registration has been approved, you may get your medication from any pharmacy. Please provide a copy of your prescription to your pharmacy.
- You will need to renew the prescription every six months with your pharmacy.
Children becoming adults
Momentum Health will charge adult contribution rates from the 1st of the month following the 21st birthday. Momentum Health makes no distinction for students or handicapped children older than 21.

Changing your personal details
It is advisable to change your address, contact and bank details directly with your medical scheme. Only changes that have a financial impact on your monthly contribution need to be made through your employer.
